It is analyzed by the local team in order to implement immediate corrective actions. 2 V Further analysis of the event (depending on its severity, its frequency, and its consequences) is carried out by local teams using the “6M/5 Why's” method to identify the root causes (technical, human and organizational factors) and define an action plan. 4 At local level, implementation of actions suited to eliminate root causes and avoid future events. 5 H%&)/0&+(?&) (24*)0&*) .-+&3(&+* A Safety-On-Line alert is sent to all EHS managers as well as General Managers of country and business concerned. All Fatalities are systematically notified to the Chief Executive Officer of Saint-Gobain, to the ExCom members and the CEOs of the country and business. 3 The event is recorded in the Group's central database (GAIA) and classified according to its rules (lost and non-lost time accident, Saint-Gobain work-related or not, type of treatment). 6 At Group level, implementation of all improvements and corrective actions needed to reduce risks; new standards proposal; lessons learned shared with all sites. 7
